Step 1: Obtaining the Certification of Cause of Death

The very first step depends on where the passing occurred:

  • In a Hospital: The attending medical officer will issue the Certification of Cause of Death (Borang JPN.LM02 / Death Slip) after confirming medical history.
  • At Home: You must immediately call the nearest police station or emergency services (999). A police officer will visit the home to inspect and issue a Police Report, permitting the body to be transferred or authorizing the doctor/hospital to confirm death.

Step 2: Registering the Death at JPN (National Registration Department)

Within 7 days of the passing, the informant (usually immediate family) must register the death at any Jabatan Pendaftaran Negara (JPN) branch or designated police station to obtain the official Sijil Kematian (Death Certificate).

Important Tip

Always request multiple certified true copies (CTC) of the Death Certificate from JPN or a Commissioner for Oaths. You will need them for EPF/KWSP, bank accounts, insurance, and estate settlement.

Step 3: Appointing a Funeral Director & Dignified Arrangements

Contact a reputable funeral service provider suited to your family's faith and cultural traditions. They can assist with body transportation, refrigeration, memorial wake venues, religious rites, and cremation/burial plots.
